As a new patient, please bring these items with you:
During your first visit with us, you can expect to receive a complete head and neck evaluation. We’ll look for potential issues, complete a dental exam, and recommend additional treatment where necessary. Our highly-trained staff will also take X-ray images of your mouth. This allows us to view the structure of your jaw, view the position of any teeth that have not yet erupted and spot any malformed roots and/or tooth decay.
If you have specific dental care needs to be addressed, sometimes it’s necessary to set a second appointment to discuss treatment in detail. Our goal is to make you feel comfortable and help you understand your options. The initial visit includes a comprehensive review of your medical history. Please provide us with complete, up-to-date health information, including recent hospitalizations, surgeries, and illnesses.
Currently taking medications? Let us know! We need the names, doses, and frequency of all the medications you take and the name of your physician. This information helps us choose the safest and most effective treatment methods for you.
