Payment in full for all services is due at the time of treatment unless personalized financial
arrangements have been made previously. We accept cash, personal check, Visa, Mastercard
or Discover.
PERSONALIZED FINANCIAL ARRANGEMENTS
We offer options because circumstances vary. Our Financial Coordinator will assist you in
selecting the option that is best for you. Pre-payment of extensive treatment (over 1500)
permits us to offer a courtesy adjustment of 5%. Lay-Away Plans make it possible to begin
treatment after payment of an agreed upon amount. Outside Financing can be arranged to
accommodate almost any monthly payment amount. Details regarding each of these options
are available by asking our financial coordinator or anyone in the business office.
DENTAL INSURANCE
With Assignment of Insurance Benefits to our office** we are able to extend the courtesy of a
grace period pending receipt of the benefits due to you from your insurance carrier. The
estimated amount not covered by your policy is due at the time of treatment unless
personalized financial arrangements have been completed previously. See the Dental

We want to help you afford the dental care you and your family need. Our financial
coordinator will answer your questions and assist in meeting your finance needs. We want you
to feel comfortable with our dental care and with your financial arrangements.
THE SMALL PRINT
All amounts 60 days past date of service are subject to service charges of 1.5% monthly
unless these amounts are part of a personalized financial arrangement. This includes any
amount not paid by an insurance carrier. Any payment due as part of a personalized payment
plan is subject to service charges of 1.5% monthly the day after the due date of said payment.

office and verification of coverage
